Does Breast Tissue Density Affect Breast Augmentation Results?

If you have had a mammogram, chances are you may be familiar with breast density and what it means for mammograms and breast cancer screenings. But what does breast density mean when it comes to breast augmentation? Will denser breasts affect your ability to achieve your desired outcome and affect your results? Here, we take a closer look at what breast density is and its effects on breast implants.

What Is Breast Density?

Breast tissue is made up of fat, fibrous tissue and glandular tissue. The ratio of these tissues is different for every woman, with some having more fat than others. Brain density is a radiologic term used to identify this ratio. Breasts with more fatty tissue allow for a clearer view of the breast by mammogram while more dense breast tissue makes it more difficult to see potential masses. Breast density is broken down into four categories:

  • Category A (Fatty) – Breast is composed mostly of fatty tissue, allowing for a clear image of the breast.
  • Category B (Scattered Fibroglandular) – Breast is mostly fatty tissue with scattered areas of glandular and fibrous tissue.
  • Category C (Heterogeneously Dense) – Higher ratio of glandular and fibrous tissue compared to fat.
  • Category D (Extremely Dense) – Breast is mostly composed of glandular and fibrous tissue, making it more difficult to see masses.

On average, at least half of all women fall into category C or D, meaning they have dense breasts. Often this means that they may have a higher risk of breast cancer and your physician may recommend additional breast screening tests, such as 3D mammography, ultrasounds, or MRIs to aid in breast cancer screenings and mass identification.

How Breast Density Affects Breast Augmentation

When it comes to breast augmentation and breast implants, breast density is not really a concern. During a breast augmentation, breast implants are placed underneath the breast tissue, not within it, meaning the breast tissue sits above the implant and does not affect the results of your augmentation. In fact, a recent study shows that there is no statistical difference in breast augmentation results between the four different density categories.

Breast density might, however, slightly affect the best breast implant placement option for you. Women with very little fatty tissue, such as those in Category A or B, have less natural cushioning over the implant, which can make the edges of the implant slightly more visible or palpable. In these cases, your surgeon may recommend placing the implant beneath the chest muscle rather than above it, as the additional tissue coverage can create a more natural-looking result. Women with denser breasts in Category C or D typically have more tissue to work with, which can help the implant blend more seamlessly with the surrounding breast. Your surgeon will evaluate your tissue composition during your consultation and factor it into their recommendations for implant size, profile and placement.

Will Breast Implants Affect Future Mammograms?

If you already have dense breast tissue and require additional screening, you may be wondering whether breast implants will further complicate the process. The good news is that mammograms are still safe and effective after breast augmentation, but there are a few things to keep in mind.

Radiology technicians use a technique called Eklund displacement views, in which the implant is gently pushed back against the chest wall so that more of the natural breast tissue can be imaged. This allows for a clearer picture than a standard mammogram view would provide with implants in place. It’s important to always let your mammogram technician know you have implants before your screening begins so they can use the appropriate technique and ensure the most accurate results.

For women with Category C or D density who are already recommended for supplemental screening, implants typically don’t change that recommendation. In many cases, MRI is actually quite effective at imaging breast tissue in augmented patients. Your surgical team and your primary care physician can work together to make sure your screening plan remains thorough after your procedure.

What about Breast Implants and Breast Cancer Risk?

This is one of the most common concerns among patients considering breast augmentation, particularly those who already know they have dense breast tissue and a potentially elevated cancer risk. Studies suggest that breast implants alone do not increase the risk of breast cancer.

However, it’s worth noting that there is a rare condition called breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), which is a type of lymphoma that has been associated with certain textured implants. The FDA and plastic surgery organizations continue to monitor and research this condition, and your surgeon can discuss your individual risk and implant options with you during your consultation.

The most important takeaway is that having breast implants does not mean you should alter or skip your regular breast cancer screenings. Staying current with mammograms and any supplemental imaging your physician recommends remains just as important after augmentation as it was before.
